To notify your COBRA coordinator of any changes within your plan, just fax the Enrollment/Change of Status (ECOS) form to us and your COBRA Coordinator will determine if the event qualifies for COBRA. If so, they will send the necessary notifications within the allotted time in order to stay compliant with the law. Examples of some qualifying events include: new hire, termination, death, marriage, and divorce.
